What does Champix actually do? It targets both "wanting to smoke" and "smoking without satisfaction" at once.

Even people who've heard the name Champix can rarely explain what kind of medicine it actually is. Hearing "stop-smoking medicine," people tend to picture "a drug that makes you hate tobacco" or "a drug that makes the urge zero" — but that's a little off.

Champix's active ingredient is varenicline. It's a drug called a nicotine receptor partial agonist, acting on the α4β2 nicotinic acetylcholine receptors in the brain. Put simply, it eases the withdrawal symptoms when nicotine stops coming in, while making it harder to get as much satisfaction as before if you do smoke. The FDA label also explains that varenicline, as an agonist at the nicotine receptor, eases withdrawal while blocking nicotine binding to reduce the reward effect of smoking.

This dual nature is Champix's defining feature.

The two reasons quitting is hard

There are two reasons quitting is hard. One is that not smoking feels bad. The other is that smoking feels good. Varenicline touches both.

For example, some people smoke right after waking. Because they wake in a nicotine-depleted state, their head is foggy, they're irritable, and they strongly crave the first cigarette. Varenicline works in the direction of easing such withdrawal.

On the other hand, when you accidentally smoke one during quitting, before, you might have felt "yeah, that's good" and gone back all at once. Because varenicline suppresses nicotine's reward effect, it tends to produce the sense of "I smoked, but it wasn't as good as I expected." That's important for stopping the chain of relapse.

Where it stands in the evidence

A Cochrane review shows that varenicline is more effective than placebo, bupropion, or single nicotine replacement therapy for achieving 6+ months of quitting. It also frames that it may have comparable effect to combining multiple nicotine replacement therapies like patch and gum.

It's not "take it and you'll quit"

But don't misunderstand here. Champix isn't "a drug that quits smoking for you if you just take it."

The FDA label also states that quit-smoking treatment tends to succeed in patients who are motivated to quit and are receiving advice and support. In other words, medicine eases the brain's distress. But changing the situations where you want to smoke is the person's own life design, and supporting that is what the stop-smoking clinic does.

When talking about Champix, you shouldn't over-hype it, nor should you scare people too much.

It's a powerful option that acts on the addiction circuit.

But the lead role in quitting isn't the medicine.

While using medicine, remake the gaps in your life where smoking had crept in. That's where the essence of quit-smoking success lies.
